
Yesterday I shared my student questionnaire so I find it only apt to share my parent questionnaire as well. While there are so many things I wanted to ask my parents, I wanted to keep it short and to the point. As always, feel free to make a copy and make it your own.

Great questionnaire. Too often the children that I need the most help to understand are the ones whose parents don’t send back the questionnaire. Do you get a good response from your parents?
This is an excellent question from Bev…what is your return ratio? I have had the same results with parents not returning forms and the families I really need to hear from are the ones that remain silent. How do you encourage parents to return the form?
My return rate is usually 100%, however, we aso used to have orientation day where parents would have time to fill it out. This year I will be mailing it out to parents with my welcome letter and then also give them time during our open house. Usually we don’t have much of a problem getting them back, however, if we don’t, I do think that also tells a story in itself.
I have the luxury of a small classroom population (maximum of 18), so I have a short, informal meeting with all my families face to face and I will certainly be adding some of your questions this year – thanks for your openness 🙂

I love this questionnaire – a really effective way of encouraging parent involvement from the very beginning of the year and of obtaining some really useful information. As I work in a PYP work I may well adapt some of the questions to include IB learner profile or PYP attitudes, but I do think the content of the original questionnaire is already excellent. Thank you for sharing!
Great idea – I typically do a “letter to me” with the same types of questions. However, this year I’m giving parents a choice – letter or google form to fill in. Good luck on a great year!
